Case Histories of Acupuncture - Down Syndrome

Introduction
In 1994 when I first came to the holistic healing centre in Bibwewadi, Pune, India, I was not sure what I would contribute. I had a background in anatomic and clinical pathology. I did a full research paper on the action of hormones on tissues and discovered that under certain hormonal conditions, the tissues in the body undergo a change which can be seen microscopically. For 24 years, I had worked in a leading hospital in Delhi, as a clinical and an anatomic pathologist and for the last 10 years as the medical director.

I am a cancer survivor, and so was advised not to practice Pranic healing or Reiki as I may be affected by the patient’s vibrations.  The only option left was acupuncture. I trained in acupuncture and started a small acupuncture healing centre. I had success with paralytic patients, and quite a few patients with arthritis of various joints.

One day my sister, who was the principal of a large school an hour away from the centre, requested me to do acupuncture on a child who was in her school: a girl with Down’s syndrome.  I explained to her that Down’s is the result of the special genes the child has and nothing can be done to change those genes. For 3 months, I was able to ward her off from what I thought was a futile experiment.

However one day the parents and the child arrived at the healing centre and requested acupuncture—saying they were fully aware that no change may take place, but they wanted to try acupuncture for her. They had already taken her for homeopathy and to a pediatrician and realized that nothing could be done. They were not able to keep the child in school and were up against a blank wall. The girl had all the characteristics of a Down’s, but in a greater degree.

I started acupuncture but could not complete the full 10 day course of acupuncture, as on the 6th day, I was called urgently to my old hospital to sort out a difficult problem. After this I completely forgot about the child till 3 months later, when her parents returned, reporting marked changes, and requested another acupuncture session.

The acupuncture sessions were continued on a yearly basis, with unexpected and remarkable results.

This child that had little or no spontaneous activity, and could only say a few unintelligible words—was now speaking freely, socializing with friends, and had expression on her face.

This experience prompted us to do acupuncture on more children with Down’s syndrome, and later also on other challenged children: those with dyslexia and autism. The following paper is based on our experiences.
A detailed discussion of our observations in the case studies is followed by discussion and hypotheses that seek an answer to the burning question:

Why and how does acupuncture help these children?

Children with Down’s Syndrome

Clinical Observations
HISTORY

In 1866, John Langdon Down described the features of these children and referred to them as Mongoloids as they resembled the Mongolian people. In 1960, this terminology was dropped and replaced by the term Down’s syndrome after John Langdon Down.

In 1959, Jerome Lejeune in France and Patricia Jacobs in England discovered in their chromosome study that these children had 3 copies of chromosome 21 and termed it Trisomy 21.They ascribed the condition of Down’s syndrome to be due to this Trisomy 21.

The test for chromosome study is called Karyotyping.

A great deal of research is being done to ascribe the characteristic features of these children to a particular gene. To date, no gene has been fully linked to any features associated with Down’s syndrome.

A few points for consideration are:

  1. 5% of children with Down’s syndrome have a normal chromosome pattern.
  2. In the 95% of children with Trisomy, this Trisomy is not inherited, but caused by a mistake in cell division during development of the ovum, sperm, or even the embryo.
  3. The second child may not be affected.
  4. The second child has a 12% risk to be affected when chrosome21 is translocated to chrosome14 in the mother (Robertson translocation).
Features of children with Down’s syndrome

  1. A flat face with upward slanting eyes and with epicanthic folds, giving the mongoloid appearance.
  2. A flat bridge of the nose.
  3. A small mouth and a protruding tongue.
  4. A small head with unusually shaped ears.
  5. Short broad hands and a single palmar crease.
  6. Broad feet with short toes.
  7. A short neck.
  8. Poor muscle tone with hyper flexibility of the joints.
  9. Lower than average cognitive ability.
  10. Lowered immunity resulting in several disease conditions.
  • Respiratory diseases are a constant problem for these children. They have regular colds, coughs and chest congestion.
  • Thyroid dysfunction.
  • Intestinal problems.
  • Heart defects especially ASD (Atrial Septal Defect).
  • Hearing loss.
The frequency is approximately 1 in 800 live births. About 6000 Down’s children are born annually. All nationalities are affected.

As a result of better understanding of the condition and supportive measures, these children live longer than they did before. But they run the risk of obesity as they reach adolescence and also of Alzheimer’s disease setting in early.

Growing awareness of the condition, has resulted in institutions for support of these children and their parents in every country.

INDIA

There is the Down’s syndrome federation at Mathru Mandir in Chennai. This was started by Mrs. Rekha Ramachandran in 1984 with 6 children and now is a global venture extending a supportive helping hand to children all over the country. Rekha‘s own daughter Babli was a Down’s child and so the institution grew around her. Dr. Rekha is a member of the international federation for Down’s syndrome.

In all these centers and also at Mathru Mandir, treatment according to the child’s needs, consists of:
  • Physiotherapy to overcome hypotonia
  • Speech therapy
  • Occupational therapy
  • Counseling
  • Yoga
  • Special nutrients and megavitamin therapy
Various specialists are attached there as needed—psychiatrists, medical specialists of all types, physiotherapists, etc. Other treatments would depend on the individual needs of the child as for colds or digestive problems and for heart problems .There is no mention of any complementary therapy except in Bangladesh and there is only a reference to it.

At this point I refer the reader to my background as an allopathic pathologist and my surprise at seeing changes in the first child with Down’s syndrome after 6 days of acupuncture. Here follows a detailed description of these changes in that child.

 Before treatment, she had:
  • A severe and expressive language disorder.
  • A protruding tongue.
  • Continuous dribbling and enuresis.
  • She could say a few unintelligible words, and had little or no spontaneous activity.
  • Her karyotyping showed Trisomy.

Three months later, her parents reported marked changes.
  • Her face had changed and now she has expression in her face.
  • Her enuresis and dribbling had stopped.
  • Her comprehension improved.
  • She was vocal and communicating with others, which she never did before.

One year later a repeat of a full 10 day acupuncture session resulted in the following changes.
  • Clearly understandable speech
  • No fear
  • No enuresis

A repeat of the 10 day acupuncture session done 1 year later showed the following changes.
  • Her aggression has gone.
  • She speaks freely and can be understood.
  • She is socializing with friends.

Methods used in our healing center in Pune

The main method is acupuncture.

Acupuncture is the science of rebalancing life energy through out the body. The energy flows through special channels called the meridians. There are 12 meridians and these are connected with the organs in our body. If there is a problem in any organ, there will be an impairment of energy flow in the corresponding meridian. Each meridian also has an emotional attitude related to it. There are specific points on each meridian where the negative energy collects. These points are stimulated by very fine needles, which are inserted in these points. Other methods of stimulation can also be used.

The main method is acupuncture using fine steel needles which are delivered to the acupuncture points using a guide with a spring .Using the guide minimizes any possible pain from the needle prick. The needles are attached to an acustimulator powered by a 6 volt or 9 volt battery. The acustimulator is kept for 1\2 an hour. Usually a few points from each meridian are given. For speech and the tongue special points around the mouth are given together with Broca’s area.
The acustimulator is set up before hand by marking the beginning of the electrical stimulation on the instrument with a special marker using another child who is older. At 1, 2 or 3 years, the children will not be able to let us know the beginning of the electrical stimulation .So we mark the stimulators before hand. We have noticed that when the stimulator is put on, the child is quiet and even falls asleep.

Sedation before the acupuncture is used to quieten the child. We normally used Zapiz (Clonazepum) as it is a mild sedative, and the strength would depend on the age of the child. A painkiller relaxes the child. And often it is a better option to a sedative. After a while the children do not need a sedative or a painkiller. Some do not need either one from the start of treatment.

We suggest 2 sessions annually for the first 2 years. Each session is 15 days. Then 1 session annually till just past puberty or 18 years.
However this would depend on the response of each child to treatment

If there is a hesitancy to use needles, there are various other options today such laser, tapping and electrical gadgets. The main principle is stimulation to the acupuncture points.

Additional Therapy

Homeopathic medicine
For aggression—Absinth, Aethusa cyan, Agaricus Muscaris, Belladonna
For sadness—Aurum met and aurum sulf
For colds—Bellis Perennis, Euphatorium Perforatum, Justica Adhota, Teucrum marum, Wyethia.

All the children need speech therapy and counseling. This is provided by other centers.
From my experience, all the mothers need guidance and counseling as well.

Each child’s story follows with the number of sessions given, and the changes seen after each session. The mother’s history during that pregnancy is also included.

 Treatment

 Session 1

(March 18th – March 28th 2002)

Inspite of heavy sedation, this child was awake and fighting the acupuncture. Only a few needles could be given on the back. She fought the needles and even pulled them out, in spite of 5 staff members holding her.
However as the days went by, she was much better.

MUMTAZ was brought to our centre on April 16th2002. She was 3 1\2years old. She was born on September 2nd 1998. Her chromosome study in January 1999 showed Trisomy. She was being treated by a homeopath that only treated mentally challenged or handicapped children. After a year’s treatment she did not show much change so her mother brought her to our centre as she heard about us.

 Mother’s history during the pregnancy

The mother reports that she was taking auto immune injections for warts on her face, and one day she realized she was pregnant. Though she stopped the injections when she was aware of the pregnancy, she had no idea how long the injections continued during pregnancy. She did not tell her obstetrician about the injections. She vomited during the full 9 months, but took no medication .She lay in bed during the full 9 months.
Vomiting in my experience is a sign of stored anger.

  • She was constantly getting colds and coughs.
  • Her IQ was 53

sessions

 Treatment

 Session 1

(April 2002)

She took 10 days of acupucture. For the first 3 days, she smiled with every needle prick; showing complete lack of awareness of what is happening to her. A pediatric nurse was giving her the acupuncture, and she was thoroughly surprised to see her smiling at every needle prick.

By the 4th day of acupuncture, the following changes were seen by her mother and us:
  1. She had lost weight and looked slimmer (she probably shed the water her tissues were holding.).
  2. Her face became smaller, lost its vacant look, and was expressive.
  3. She was now aware of her mother and all of us.
  4. She was also aware of pain with each needle, as she was crying after each prick. This was a significant change.
  5. Her aggression was now apparent.

 Sessions 2, 3

(September 2002, November 2002)

The following changes were reported by the mother:
  1. Mumtaz was now talking in the house and understood what was being said by those in the home.
  2. Her memory seemed to have improved as seen from her behavior.
  3. She was now able to stand up to her older brother and fought back.
  4. However she was a little more selfish than before and was not sharing her toys with others.
  5. Her stools started to give an abominable stink. (This is common to all Down’s children as they improve.)

 Sessions 4, 5

(April–May 2003, October–November 2003)

The mother reported the following after this session:
Her extreme shyness was slightly reduced and she seemed more sociable than before.

 Sessions 6, 7

(September 2004, November 2004)

Her mother reported:
  1. She spoke more clearly now and answered others appropriately.
  2. Her neck however was quite stiff and she kept her head bent while walking.
  3. She now had a normal face and no more looked like a Down’s child for which the family was most grateful, especially her father.
  4. She still had a great deal of fear. If she saw a dog even at a distance, she runs away. In the centre, she did not look at us but put her face down.

 Sessions 8, 9

(April 2005, November 2005.)

  1. Her speech was clearer and she expressed herself better.
  2. She was independent now; took care of herself and went to school in the bus by herself.
  3. She was doing well in school and her handwriting was very clear. She is in a school for normal children.
  4. She grasped what was being said and answered appropriately.
  5. She was more sociable than ever before and expressed her feelings appropriately.

 Sessions 10, 11

(May 2006, October–November 2006)

  1. She looked normal now and there was no doubt about that.
  2. She kept her head up and looked at others, and was not as shy as before.
  3. She is only now bonding with her mother. Until this time, she was only close to her father and her brother and did not relate to her mother.

 Sessions 12, 13, 14

(2007, April 2008, November 2008 )

  1. She has grown up, but is still a bit shy and has an abnormal degree of fear.
  2. With speech therapy, she spoke clearly and freely.
  3. She looked normal now, and does not have the features of a Down’s child.
Remarks:
  1. The family stays in a remote village where there is only one school and no facilities for such children. For counseling and speech therapy, she comes into our city (Pune or Mumbai) and only twice a year when she comes for her acupuncture.
  2. The mother does not want to put Mumtaz into a school for mentally challenged children as she feels the girl will only see children like her and will not change. In a normal school, she faces challenges that stimulate her.
  3. Mumtaz was shown her first photo before treatment and she refuses to accept the fact that she ever looked like that. Her mother tried to tell her that the photo was taken before treatment was started in the Bibwewadi centre. But the child says we have put someone else’s picture there.
  4. Her mother called to say she has no more fear now but does not care to socialize. This may be a family programming as the parents also do not socialize in that remote village.

JOHN DOE came to our centre at 5 ½ years on February 16th 2004. His chromosome analysis showed classical Trisomy21. At present, he is attending a school for retarded children.

 Mother’s history during the pregnancy

 Mother’s history

We noticed that the mother herself was slightly mentally retarded. She told us that she is an orphan and does not remember her own mother. As a young child, she did menial work for a family and was badly beaten and traumatized till another family saved her and eventually got her married.

 Mother’s history during the pregnancy

Early in her pregnancy, she started bleeding per vaginum (P V) and a doctor treated her with a hormonal medication and did not tell her she was pregnant. When she changed her doctor, she was told she was pregnant and was asked to stop all the medicines. She has 2 older girls who go to school. She is unable to tell us any more than this.

She was able to take the acupuncture for 10 days. For the first 3 days, only the back points were given on the UB meridian.
By the 3rd day, Naomi was aggressive and restless. In spite of sedation, her energy needed 2 persons to hold her.
For the next few days, she took acupuncture but was restless during the acupuncture, in spite of sedation.

By the 10th day before she left for home, the following changes were seen by all those who were with her in the centre and those with whom she was living:

  1. She was now eating porridge and also some bread.
  2. She had started eating rice and fish which she refused before.
  3. She also drank water.
  4. She held her head up straight.
  5. The dribbling had stopped and her mouth was closed.
  6. The face had changed and was expressive, not empty and blank.
  7. She was aware of her surroundings and was alert, and responded to calling and to those around her.
  8. The face had changed and was expressive, not empty and blank.
  9. She was able to follow instructions and ask for what was needed.
  10. She was now able to stand and also to walk though unsteadily when she holding hands with persons on either side of her. She was only able to sit up before acupuncture.
  11. These changes were seen before she left for home, and the mother was grateful, and said she would bring her as often as needed.

I really began to be more aware of a mother’s feelings when I saw the children show recognition of the mother after the first acupuncture session and the mother’s joy.

 Session 2

(July 31st–August 11th 2006)

During this session, she was quiet and cooperative, and needed minimal sedation.

The mother reports the following changes have taken place in the last 3 months after her 1st acupuncture session:

  1. The child was now able to walk and run entirely on her own.
  2. She was able to drink by herself from a cup, and eat normally.
  3. She was friendly with other children and quite sociable.
  4. She responded to people and situations appropriately.
  5. A repeat echocardiogram at the same centre showed the heart was within normal limits.
  6. Her face was normal and she did not have the features of a Down’s child.
Remarks:

To our dismay, the child did not come back as the treating pediatrician told the father that the child would die early if acupuncture continued.

We need to remember that these are poor villagers who would not think of challenging the pediatrician, but would rather look up to him.
We have had a message from people in the neighborhood however that the child is going to school and is happily adjusting.
.

Unfortunately for this child, both the parents are working—the mother for 12 hours and the father for 16 hours and there is no one to bring the child for further treatment. So they stopped the treatment and went for physiotherapy once a week on Saturdays.

MOSES was brought to our centre in January 2007 at 3 years of age. His chromosome study showed Trisomy

 Mother’s history during the pregnancy

This pregnancy was unexpected, and came at a time of financial crisis. The husband had to resign as he was not paid for months. Her father had expired and she was having difficulties with her brother, and was very angry at this time. Her husband discussed the possibility of abortion, but she decided against abortion.
There was difficulty, sadness and anger during the first 3 months of pregnancy. But in the 4th month, she pulled herself together and started to sing and talk to Moses.
However in the 7th month, she undertook a difficult journey to the south of India from Shillong. When she returned, her doctor found the child’s head engaged and advised total rest. This was not possible for many reasons. By the 8th month, the child was delivered by cesarean section and the mother was hemorrhaging profusely and not expected to live. However she survived. The child could be brought to the mother only after 3 days for breastfeeding.

We lost touch with this child as the family moved to another state where he was put in school with other children for stimulation. Due to time constraints, the mother was unable to bring him to the centre.
Eventually we were sad to hear the child died of a severe cold and chest infection.

ROGER was brought to our centre in March 2008 .He was 11 years old. His chromosome study showed Trisomy21. He was delivered by cesarean section at 7 months gestation as the umbilical cord was around his neck.
